The problem was that the recommendations AI provided were based heavily on limited data sets. You need real world patient data, which happens to be very complex. Not only that, but the workflows were disconnected. The AI wasn’t embedded into the daily routines of doctors, so the suggestions often felt off-putting or unsafe. That forced clinicians to interpret or ignore the tool rather than collaborate with it. Because the feedback loop from clinicians to developers was finicky, user trust eroded fast.

Within the first two weeks, the system was already doomed to fail. For a system with over 24,000 unique payroll combinations, just two weeks were spent gathering “critical business requirements.” Nurses, doctors, and admin were all governed by a mess of industrial agreements, allowances, and exceptions. The system was never built to reflect that level of complexity. Despite thousands of known bugs and zero end-user confidence, Queensland Health went live with the system. When problems hit the payroll teams and frontline workers, they were the ones holding the system in place, but no one thought about including them in the rollout. They didn’t have any tools or training in the new system, and trust was eroding faster than a sandcastle during high tide.

In other cases, the AI continued to accept orders for items that were out of stock, creating confusion and frustration at the window.

After 3 years of development with IBM, McDonald’s pulled the plug in June 2024, proving that even tech backed by time, money, and brand muscle can collapse without the right operational scaffolding beneath it.
